Treatment for Aldosterone
How you treat either high or low aldosterone will vary from person to person. Even those with low aldosterone many not need Florinef.
The most important factor in treating aldosterone is being able to get sodium and potassium labs done every 3 - 4 weeks. Supplementing with Celtic sea salt and usually slow release potassium are essential when treating low aldosterone.
Aldosterone affects the Diastolic Blood Pressure and often when systolic pressure is good (110-130) and diastolic pressure is low (below 70) it is caused by low aldosterone so this is also a measure of correct mineralcorticoids.
To begin Fludrocortisone (Florinef) you will want to go very slowly as it is a very potent steroid and changes your body's fluid balance a lot. I generally recommend 1/4 tab to begin with and only raise to 1/2 tab after 7-10 days depending on reactions and severity of problems. It is raised in this manner, 1/4 tab at a time, until the BP is normal and low aldosterone symptoms disappear. If at any time BP goes too high it should be reduced. It can also cause fluid retention and pressure headaches, so these are signs of too much, or possibly it is lowering potassium too much so this is why electrolytes need to be checked often as you adjust this.
High Aldosterone is not seen very often. It is treated by first testing and adjusting sodium and potassium levels. If this does not help the next step would be to take medication. This could be a high blood pressure medication such as ARB's (angiotensin receptor blockers) or stronger medications like Spironolactone or Eplerenone. These medications need to be closely monitored via blood test. This blood test should include:
- RBC Potassium
- RBC Magnesium
- Kidney Function Test
If these medications do not lower your high aldosterone symptoms, then tests should be done for an adrenal tumor.

Tracking your Aldosterone Treatment
Charting your blood pressure while sitting and then standing along with sodium and potassium labs and charting temps is how you track your aldosterone treatment.
Make sure that you have been resting for at least 15 minutes and have not had anything to eat, drink or smoke. Take your blood pressure sitting and then immediately standing. Make certain your arm is at heart height! When aldosterone is at a good level your systolic number should rise when standing. 10 points is preferred, but not falling is vital. This can also be affected by cortisol levels so it can be tricky to figure out which is doing which.
Sodium level should be 142-144
Potassium level should be 4.0-4.3
